The Transition Manager will play a critical role in a major NHS transformation programme, overseeing the transition strategy from bid and mobilisation to operational handover. They will lead the design and implementation of processes that ensure successful service delivery and integration into operational environments, working collaboratively with various stakeholders including programme leadership and service management teams. This is a high-profile role that demands expert knowledge in service transition and operational readiness within a healthcare context.

Responsibilities

  • Support bid and procurement activities by shaping transition and operational readiness strategies.
  • Develop and implement programme-to-BAU transition plans and governance frameworks.
  • Establish operational readiness, service acceptance, and go-live assurance processes.
  • Lead service introduction activities across multiple workstreams.
  • Define and govern hypercare, early-life support, and service stabilisation arrangements.
  • Coordinate supplier onboarding and integration activities.
  • Drive knowledge transfer and training efforts during operational handover.
  • Conduct service acceptance reviews and readiness assessments for successful go-live decisions.

Requirements

  • Extensive Transition Management experience in large-scale transformation or business change programmes.
  • Proven track record of transitioning complex services into operational environments.
  • Strong expertise in Service Introduction, Service Transition, and Operational Readiness.
  • Solid understanding of ITIL principles and Service Management best practices.
  • Experience within multi-supplier delivery and support environments.
  • Demonstrable experience establishing and leading transition governance frameworks.
  • Excellent stakeholder management and leadership skills.
  • Desirable experience in NHS transformation programmes or complex support ecosystems.
